A surrogate endpoint is a measurement used to stand in for an outcome that matters directly to patients, such as using blood pressure in place of stroke. Surrogates allow smaller, faster trials, but only work if the substitute reliably predicts the real outcome. History includes cases where a surrogate improved while patients did worse.
