The monitoring plan describes how a specific trial will be monitored: the mix of on-site, remote, and centralised activity, the visit frequency, and what will be reviewed at each. Modern plans are risk-based, so effort is concentrated on the data and processes that matter most. It is written before the first site opens and updated as risks change.
