A protocol amendment is a formal, documented change to an approved protocol. Substantial amendments — those that could affect participant safety or the reliability of the results — normally need ethics committee and regulatory approval before they take effect. Sites must not implement a change until the required approvals are in place, except to remove an immediate hazard.
