Source data review is reading the site's records as a whole to judge whether the trial is being run properly — not just whether individual values were transcribed correctly. It looks for unreported adverse events, eligibility problems, missed assessments, and consent issues. SDR and SDV are complementary: review asks "is this right?", verification asks "does this match?".
